Oof, I've done a lot. Luckily I've never broken the Internet but I certainly automated things on network devices that could definitely do that....
But I've done all of the normal stuff: I've deleted more data than I wanted in databases, I've released code that actively corrupted data, I've released memory leaks, approved PRs that I probably should have held back for more review, etc. etc.
